Fusion of the prepuce over the glans clitoris and loss of the labia minora, along with whitened plaque and crinkled whitened skin, with patches of post inflammatory hyperpigmentation indicate lichen sclerosus.

Lichen sclerosus can present with subtle changes in the skin: Note the asymmetry of the labia minora, with flattening on the patient’s right side. Note whitened skin on the right labium minus. Note the thin prepuce that is shrinking back, exposing the glans clitoris. Note the thickened white skin in the perineal area.
